Residence: Building B16, General Staff Residential Area, Lane 699, Lac Long Quan, Tay Ho District, Hanoi.

Member of the Communist Party of Vietnam, Deputy Commander, Nghe An Provincial Military Command, Military Region 4.

Glorious Soldier Medal, First, Second, Third Class; Victory Flag Medal; 30-year Party membership badge.

He passed away at 22:37, December 18, 2016 (November 20, Binh Than year) at 108 Central Military Hospital, due to an accident.

The funeral will be held from 8:00 - 10:00 a.m. on December 24, 2016 at the Ministry of National Defense Funeral Home (No. 5 Tran Thanh Tong, Hanoi).

The memorial service and funeral will be held at 10am the same day; burial will be held at Tu Lien Ward Cemetery, Tay Ho District, Hanoi.
