Recently, HV General Hospital (Phu Tho) received a case of patient NHG (14 years old, Na Hang, Tuyen Quang) in a state of passing fresh red blood, abdominal pain, vomiting black blood, painful swelling of the knee and ankle joints, nausea, and red rash all over the body. From the results of blood tests and typical clinical manifestations, doctors diagnosed the patient with allergic vasculitis (Henoch Schonlein) leading to gastrointestinal bleeding.

Allergic vasculitis is an autoimmune disease that causes inflammation of the small blood vessels in the body. The disease is common in children aged 3-15 years old, occurring year-round but peaking in winter and spring, when the patient has a cold, respiratory infection, allergy, etc. The disease is easily confused by parents with symptoms similar to skin diseases, so it is very dangerous if self-treated at home.

Patient G. had typical symptoms of allergic vasculitis but was not hospitalized for timely treatment, leading to severe gastrointestinal bleeding. After being hospitalized for treatment in the Emergency Resuscitation Department, the patient was stable and transferred to the Internal Medicine - Pediatrics - Oriental Medicine Department for monitoring.

Typical symptoms on the body when sick:

- Skin symptoms: red rash like mosquito bites appears densely or scattered on both calves, around the ankles, buttocks, thighs, forearms, and the whole body.

- Joint symptoms: swelling and pain in the knees, ankles, wrists, hands, fingers, and elbows. Pain is mostly in the knees and ankles.

- Gastrointestinal symptoms: abdominal pain is a common symptom, accounting for 70-80% of cases. Dull pain around the navel or pelvis, sometimes severe pain. Gastrointestinal bleeding, vomiting, bloody stools, black stools, possible complications of intestinal obstruction, intussusception, infarction, acute pancreatitis.

- Kidney symptoms: patient has hematuria, edema, may develop glomerulonephritis, chronic renal failure, nephrotic syndrome.

Children with the above symptoms should be taken immediately to a specialized medical facility for timely examination and treatment.

During treatment, it is necessary to follow the doctor's instructions, do not let the child run, jump, or do strenuous activities that cause hemorrhagic rash to appear. The disease recurs many times leading to kidney failure, endangering the child's life./.
