Below is information on conditions, subjects, procedures, places of issuance, and exchange of new health insurance cards.

1. When will the new health insurance card be issued?

When local social insurance agencies run out of blankshealth insurance cardOnly old model health insurance card will be issued new model.

2. Who is the new health insurance card issued to?

After the provincial Social Insurance has used up all the old health insurance card templates, they will issue new health insurance cards for the following cases:

- New participants in health insurance.

- Participants lost their health insurance card.

- The participant's health insurance card is torn, damaged or has changed information.

* Note: During the transition period from the old health insurance card model to the new model, those who are using the old health insurance card model with valid validity period can continue to use it for health insurance examination and treatment and extend the validity of the health insurance card based on the management database, do not print or change the health insurance card (except for the above cases).

3. Place to receive application for new/reissued health insurance card

a) At the Social Security agency in the following cases:

+ People whose health insurance is paid by the social insurance organization.

+ People whose health insurance premiums are partially supported by the state budget.

+ People participating in health insurance by household.

+ People receiving unemployment benefits change their health insurance cards.

b) At the Social Insurance and Health Insurance Collection Agent in the following cases:

+ People whose health insurance premiums are partially supported by the state budget.

+ People participating in health insurance by household.

c) At the People's Committee of the commune in the following cases:

+ People whose health insurance is paid by the state budget.

+ People whose health insurance is paid by the social insurance organization.

d) At the Enterprise:Health insurance participants pay at the enterprise and submit their application to the employer.

4. What procedures and documents are required when registering to change cards?

- Participants fill out the Social Insurance and Health Insurance participation declaration and information adjustment form (Form TK1-TS);

- For organ donors: Additional discharge papers stating "donated organs";

- In case the participant is entitled to higher health insurance benefits: Provide supporting documents (if any).
