Looking back at 15 years of implementing Directive 38-CT/TW on "Promoting health insurance in the new situation"
After 15 years of implementing Directive 38-CT/TW dated September 7, 2009 of the Secretariat of the 10th Party Central Committee on "Promoting health insurance in the new situation", Nghe An has deployed many synchronous and drastic solutions and achieved many remarkable results. In particular, the health insurance coverage rate is increasingly high, affirming that health insurance plays an important role in people's lives, contributing significantly to the implementation of health care goals and ensuring social security in the area.
The whole political system got involved
The most obvious change in Nghe An is that before Directive No. 38 was issued, information and propaganda activities on health insurance policies were mainly undertaken by the Social Insurance sector. By 2010, especially from 2015 to now, the entire political system has been involved in informing and propagating health insurance policies in many diverse forms (through newspapers, radio, grassroots radio systems; billboards; slogans; leaflets; brochures; CDs; through reporters' conferences, dramatized forums...); at the same time, orienting public opinion against false information on health insurance policies. Changing the form of propaganda from monologues at conferences to direct dialogue, bringing health insurance policies to a large number of people in each village, hamlet, and residential area.
Maximize the role of the electronic information portal of the Social Insurance and Health sectors to provide people with official information on policies, mechanisms and policies related to Health Insurance. Use social networks to strengthen information and propaganda work on Health Insurance policies in line with the trend of the information technology and digital technology era. Organize propaganda conferences and dialogues with businesses, through working sessions between Social Insurance officers and units; through hotlines and business support consulting teams of the Provincial Social Insurance.
Propaganda at working sessions between labor inspectors, Social Insurance and debtors (at least one working session per month), such as sanctions for violations of the law on paying Social Insurance, Health Insurance, and solutions to overcome difficulties. Organize dialogues with workers at workplaces and boarding houses so that workers clearly understand their rights, thereby taking actions to fight with employers to fulfill their obligations to ensure the rights of workers in participating in Health Insurance.
As one of the collection service staff who does a good job of propaganda and expanding the group of people participating in health insurance, Ms. Nguyen Thi Lien - President of the Women's Union of Trung Do Ward (Vinh City) said that the "key" for people to understand the benefits of health insurance is propaganda. But how to propagate and how to propagate is also a secret that every collection service staff needs to have. For Ms. Lien, propaganda is not only through conferences, through block meetings... but propaganda here is to go to the house, understand each family's situation to advise them on the most reasonable payment methods.
To effectively implement Directive 38, the Provincial People's Council and People's Committee have also issued many mechanisms and policies to support health insurance participants in each phase. Strengthening the inspection, supervision and monitoring of the implementation of health insurance policies at all levels, sectors, organizations and units, especially enterprises; overcoming and eliminating the situation of non-participation or long-term debt of health insurance payments by enterprises; regularly inspecting, supervising and monitoring the use of health insurance at health insurance examination and treatment facilities to strictly handle and limit and prevent abuse or profiteering from health insurance. Through inspection work, it shows that the quality of medical examination and treatment at health examination and treatment facilities basically meets the needs of the people.
Up to now, the satisfaction rate of patients when coming to medical examination and treatment facilities in the province continues to improve (the average satisfaction rate of patients at inpatient medical examination and treatment facilities is 97.19% and outpatient is 94.48%).
After Directive No. 38 and guidance from the Central Propaganda Department, the Provincial Party Standing Committee and Party committees at all levels issued important documents to lead, direct and organize the implementation of Directive No. 38 in localities, agencies and units. The Provincial Party Committee issued 13 documents, the Provincial People's Council issued 9 documents, the Provincial People's Committee issued 72 documents for leadership and direction; the Department of Health issued 47 documents, the Provincial Social Insurance issued 46 documents to synchronously implement tasks and solutions on Health Insurance. Party committees and authorities of districts, cities and towns issued 625 documents to strengthen leadership, direction and specify the contents of implementing Directive No. 38.

In Nghe An, as of December 2023, the health insurance coverage rate reached 93%, exceeding the 2023 coverage rate assigned by the Government.
Through that, it is seen that people are increasingly aware of the importance of participating in health insurance, which is one of the factors contributing to reducing the risk of poverty and contributing to social security.
Mr. Nguyen Ba Quang (Nam Thanh commune, Nam Dan) said: Recently, Nam Dan Social Insurance officers came to propagate participation in Health Insurance. Seeing the humane benefits of Health Insurance, my family and I decided to participate, both husband and wife.
As for Mr. Tran Van Hai (Dien Thinh commune, Dien Chau district), he not only participated in health insurance for himself but also registered for his wife, daughter-in-law, and son. According to Mr. Hai: His whole family is self-employed, no one knows in advance about illness or disease, so when he is healthy and still able to work, he buys a health insurance card to prepare for risks.
It can be affirmed that after 15 years of implementing Directive 38-CT/TW on "Promoting health insurance in the new situation", there are still many limitations: The propaganda and dissemination of Directive 38 and the Party's resolution on health insurance work have not been given due attention at times and in some places, and have not been implemented regularly and extensively; and have not reached people in remote areas and ethnic minorities.
A part of the population has not fully recognized the position and importance of Health Insurance in the social security system. The compliance with the law on Health Insurance by employers is not serious. The management and use of the Health Insurance fund is sometimes and in some places not really effective. The quality of medical examination and treatment in some medical facilities does not meet the requirements, especially at the grassroots level.
To continue to effectively implement Directive 38 and move towards universal health insurance, Nghe An's tasks and solutions in the coming time are: Focus on leading, directing, and implementing the Health Insurance policies and guidelines, especially the Campaign for the entire population to participate in Health Insurance launched by the Central Government; Party committees and authorities at all levels issue documents to lead, direct, and organize the implementation of Health Insurance policies to implement directives and resolutions, and include the rate of participation in Health Insurance in the annual and 5-year socio-economic development targets of localities and units; There are many solutions to mobilize resources to support groups of people participating in Health Insurance, in order to rapidly increase the coverage of Health Insurance, moving towards achieving the goal of universal Health Insurance.
Building a team of civil servants and public employees of the Nghe An Social Insurance system with professional qualifications, professional ethics, modernity and professionalism to build a modern, professional Social Insurance sector, aiming at people's satisfaction. The health sector strengthens training of medical human resources, invests in facilities and equipment to deploy towards building Vinh city into the core of the specialized medical center of the North Central region in the spirit of Resolution No. 39-NQ/TW, dated July 18, 2023 of the Politburo on building and developing Nghe An province to 2030, with a vision to 2045.
From there, mobilize the responsible participation of all levels, sectors and localities to join hands to increase health insurance coverage, striving to achieve the target of health insurance coverage of 95% of the province's population by 2025./.
