Nghe An General Hospital saves the life of a patient in hemorrhagic shock due to abdominal trauma and liver rupture

Recently, Nghe An General Hospital saved the life of female patient NTO, 35 years old, residing in Ha Tinh, who suffered from hemorrhagic shock due to closed abdominal trauma and grade 4-5 liver rupture.

According to the family, the patient had a traffic accident between a motorbike and a tractor and was taken to Ha Tinh General Hospital for emergency treatment. At Ha Tinh General Hospital, the patient was diagnosed with hemorrhagic shock due to blunt abdominal trauma. The patient underwent emergency intervention to suture the liver, stop the bleeding, open the gallbladder and pack the bleeding with gauze. The patient was then quickly transferred to Nghe An General Hospital for further treatment.

The patient was transported with pale skin, pale mucous membranes, blood pressure 90/60, vasopressors were used, abdominal drainage was bleeding profusely, the abdomen had a 35 cm long white line incision above the navel. Realizing the patient's critical condition, immediately after consultation between the two hospitals, grasping the patient's situation,Nghe An General Friendship Hospitalurgently prepared the surgical team to be ready to receive the patient.

Immediately after being transported to the Emergency Department - Nghe An General Hospital, the patient was quickly consulted and had emergency surgery with the participation of doctors from the General Surgery Department.

After 2 hours of surgery, the surgical team led by Dr. Nguyen Huy Toan - Deputy Head of General Surgery Department successfully performed liver resection, hemostasis, and gallbladder drainage for the patient. During the surgery, the patient was continuously transfused with 3,000 ml of packed red blood cells and 1,000 ml of fresh frozen plasma, and vasopressor infusion.

The patient was monitored and cared for for 7 days in the Surgical Intensive Care Unit. On November 23, the patient was transferred to the General Surgery Department for further treatment. The patient is now stable and has been discharged from the hospital.

Previously, on January 4, 2024, a 32-year-old male patient in Cam Binh commune, Cam Xuyen district, Ha Tinh province was also transferred to the emergency room from Ha Tinh Provincial General Hospital with a diagnosis of hemorrhagic shock due to multiple injuries of grade 4-5 liver rupture, grade 4 right kidney rupture, and right colon muscle contusion.

The patient was transferred out.Nghe An General Friendship Hospitalwith coma, breathing through endotracheal tube, pale skin and mucous membranes of eyes, blood pressure 90/60 depending on vasomotor, white line incision above and below the navel, two abdominal drains with a lot of fresh blood. The emergency department held a multidisciplinary consultation with the General Surgery Department and the Nephrology and Urology Department to perform emergency surgery for the patient.

During the surgery, selective hepatic pedicle control was used to limit ischemia of the remaining liver; hemostasis of the right renal parenchyma was sutured - right colon colostomy was created; Art Line hemodynamic monitoring devices were placed (invasive arterial blood pressure measurement); Central venous catheter was used to transfuse blood and blood products to the patient (about 3,000 ml of red blood cells and blood products).

After surgery, the patient was actively resuscitated using advanced techniques such as blood filtration combined with continuous dialysis. The resuscitation process at the Surgical Intensive Care Unit lasted 11 days. The patient was transferred to the General Surgery Department for continued treatment. After that, the patient returned home healthy to celebrate the Lunar New Year 2024.
