Nghe An General Hospital successfully saved a patient suffering from hemorrhagic shock due to abdominal trauma and liver rupture.

Recently, Nghe An Friendship General Hospital successfully saved the life of a 35-year-old female patient, NTO, residing in Ha Tinh, who suffered hemorrhagic shock due to blunt abdominal trauma and a grade 4-5 liver rupture.

According to family members, the patient was involved in a traffic accident involving a motorbike and a tractor and was taken to Ha Tinh General Hospital for emergency treatment. At Ha Tinh General Hospital, the patient was diagnosed with hemorrhagic shock due to blunt abdominal trauma. Emergency procedures included liver suturing, hemostasis, gallbladder drainage, and gauze packing to control bleeding. The patient was then quickly transferred to Nghe An Friendship General Hospital for further treatment.

The patient was transported in a pale condition, with pale skin and mucous membranes, blood pressure of 90/60, requiring vasopressors, abdominal drainage for significant bleeding, and a 35 cm long midline abdominal incision above the navel. Immediately after being transported to the Emergency Department of Nghe An Friendship General Hospital, the patient was quickly examined and underwent emergency surgery with the participation of doctors from the General Surgery Department.

After a two-hour surgery, the surgical team led by Dr. Nguyen Huy Toan – Deputy Head of the General Surgery Department – ​​successfully performed a hemostatic liver resection and gallbladder drainage for the patient. During the surgery, the patient continuously received 3,000 ml of packed red blood cells and 1,000 ml of fresh frozen plasma, along with vasopressors.

The patient was monitored and cared for in the surgical intensive care unit for 7 days after surgery. On November 23rd, the patient was transferred to the general surgery department for further treatment. The patient is now stable and has been discharged.

Previously, on January 4, 2024, a 32-year-old male patient from Cam Binh commune, Cam Xuyen district, Ha Tinh province was also transferred from Ha Tinh Provincial General Hospital with a diagnosis of hemorrhagic shock due to multiple traumas: grade 4-5 liver rupture, grade 4 right kidney rupture, and contusion of the right colon muscle.

The emergency department conducted a multidisciplinary consultation with the General Surgery and Urology departments to perform emergency surgery.

During the right hepatectomy, selective hepatic pedicle control was used to limit ischemia in the remaining liver; hemostasis was achieved by suturing the right renal parenchyma; a colostomy was created in the right colon; hemodynamic monitoring devices such as Art Line (invasive arterial blood pressure measurement) were placed; and a central venous catheter was used for blood and blood product transfusions (approximately 3,000 ml of red blood cells and blood products).

Following surgery, the patient received intensive care using advanced techniques such as continuous hemodialysis. The intensive care unit for 11 days provided the patient with the option to be transferred to the General Surgery department for further treatment. Afterward, the patient recovered and returned home to celebrate the Lunar New Year 2024 with their family.
