On May 3, Tuyen Quang General Hospital admitted patient Bui Van S., 57 years old, from Son Duong district, Tuyen Quang, with intermittent high fever of 39.5 degrees. The patient's armpit had red ulcers.

After examination, Dr. Nguyen Van Hung, Department of Examination, diagnosed the patient with scrub typhus and transferred him to the Department of Infectious Diseases for treatment.

The patient's family said that 7 days ago, Mr. S. suddenly had a high fever and severe headache. The family rushed him to the district general hospital for treatment, but there was no progress so he was transferred to a higher level hospital.

Dr. Hung said that scrub typhus is an infectious rash fever caused by the Rickettsia tsuisugamushi virus (commonly known as the mite).

This type often bites patients in hidden, hard-to-detect places such as the armpits, chest, groin, anus... After the incubation period, the virus will enter the blood, endothelial cells in blood vessels and proliferate, causing inflammation, congestion at the bite site and rash, swollen lymph nodes accompanied by headache and prolonged high fever, which can reach 40-41 degrees.

The progression of the disease varies from mild to severe, causing damage to many organs. Severe cases in the early stages will cause peripheral circulatory failure (usually appearing in the first week), late stages will have proliferative lesions and embolism appearing in small capillaries, coma, liver dysfunction... and can cause death.

In the North, the disease mainly appears in mountainous provinces, where there are many bushes and dense forests. Those who are susceptible to tick bites are farmers, loggers, and land reclamation workers...

Scrub typhus usually occurs during the hot and rainy season, from May to October.

This fever is easily misdiagnosed with other diseases such as typhoid, malaria, leptospirosis, plague, septicemia, dengue fever...

Doctors recommend that people should wear socks on their hands and feet, and wear tight-fitting clothes when going into mountainous areas or places with dense vegetation; do not dry clothes, place backpacks on bushes, or lie on the grass...
