Instructions on how to fill out the Party Membership Form completely, section by section, according to the new regulations.
The Party Membership Form (Form 2-HSĐV) is an important document used to manage party members after admission. This article provides instructions on how to correctly fill out each section according to Guidance 38-HD/BTCTW.
The concept of Party Membership Certificate
The Party Membership Form is a document issued in conjunction with Guidance 38-HD/BTCTW of the Central Organization Committee. According to Section 2.2 of this Guidance, after being admitted to the Party, members must fill out the Party Membership Form for management purposes.
The membership form must be stamped with a wet seal by the grassroots Party committee on all pages, and the photo must also be stamped with an embossed seal. This is a mandatory document after the Party branch decides to admit the member.
Those required to fill out the Party Membership Form include: party members currently participating in official party activities, party members exempted from party activities, newly admitted members, and party members transferring their official membership out of the district-level or equivalent party organization.

The current Party Membership Form has the code 2-HSĐV, issued with Guidance Document 38-HD/BTCTW. This form is used uniformly in all Party organizations.

Detailed instructions on how to fill out the Party Membership Form.
1. Items in the headings
The names of the provincial Party committee, the immediate superior Party committee, the grassroots Party committee, the departmental Party committee, and the Party branch must be clearly stated according to reality. If it is a grassroots Party branch, write the name of the branch in the line "Grassroots Party Committee/Branch".
The party member's biographical number is recorded by the Party organization that manages the party member's records.
The Party membership card number should be filled in according to the new card number as per Directive 51-CT/TW issued on August 8, 2025, consisting of 12 digits written in each box in order from left to right, for example:

Party members must use 3x4 color portrait photos. Party members in the armed forces follow separate regulations.
2. Items in the content section
(1) Full name at birth
Write exactly as it appears on the birth certificate, using capital letters, for example: NGUYỄN VĂN HÙNG.
(2) Gender
Specify whether it's "male" or "female".
(3) Other names
Write down a pseudonym or alternative name if you have one.
(4) Date of birth
Record the day, month, and year as stated on the birth certificate.
(5) – (9)
This includes place of birth registration, birthplace, current residence, ethnicity, and religion. The content is the same as the corresponding section in the applicant's Party membership application form as per Point 1.3, Section I.
(10) Family composition
Record the social class of the father, mother, or guardian during the 1954 land reform period (North Vietnam) or the 1976 worker-peasant-trade transformation period (South Vietnam). If not falling under these categories, leave blank.
(11) Current occupation
Record workers, farmers, civil servants, officials, soldiers, students, unemployed people…
(12) Party membership date and official date
Record the date the Party branch held the membership ceremony and the date of official recognition. Include the full names of the Party branch, the grassroots Party committee, and the higher-level Party committee.
Party membership recommenders: record the full name, position, workplace, and current place of residence of each recommender. If the youth union or grassroots trade union makes the recommendation, clearly state the name of the organization.
3. Item (13): Date of recruitment and stages of revolutionary activity
This section covers multiple historical periods and requires accurate recording according to regulations:
Before August 19, 1945: Record the date of commencement of activities within organizations led by the Indochinese Communist Party.
August 20, 1945 to July 1954: Record the period of participation in agencies belonging to the Indochinese Communist Party, the Vietnam Labor Party, or the system of government and mass organizations.
August 1954 to April 30, 1975:
Northern Vietnam: record the date of recruitment into an agency or unit belonging to the State system of the Democratic Republic of Vietnam.
Southern Vietnam: record the date of joining organizations belonging to the Vietnam Labor Party, the Southern People's Revolutionary Party, the National Liberation Front, or being assigned tasks by the revolution.
From May 1st, 1975: Record the date of recruitment into an agency belonging to a Party organization or a State agency.
From February 26, 1998 to December 31, 2009: Record the recruitment date according to the Ordinance on Cadres and Civil Servants.
From January 1, 2010: Record the recruitment date according to the Law on Cadres and Civil Servants.
From January 1, 2012: Record the recruitment date according to the Law on Public Employees.
4. Section (14) – (30): Additional information
Day of joining the Youth Union (14)
Record the date of admission to the Ho Chi Minh Communist Youth Union.
Social organization participation day (15)
Record the dates of joining organizations such as women's associations, trade unions, veterans' associations, etc.
Enlistment and discharge dates (16)
Record the full date of enlistment, highest rank held, and date of discharge or re-enlistment.
Current level (17)
Record your professional qualifications, political theory knowledge, and foreign language skills.
Health status (18)
Indicate good, average, or poor. If a war veteran, indicate the type of injury.
ID number (19)
Record according to your current citizen identification number.

Note regarding verification of Party Membership Certificates
The grassroots Party committee must affix a wet seal to all pages and a raised seal to the photo on the form. Party members must complete the form within 30 working days of their admission for Party organization management.
