The firstRegarding basic documents, the person registering the birth must submit:
Birth registration form according to the prescribed mẫu (template).
Birth certificate issued by a medical facility.
If a birth certificate is unavailable, it can be replaced by a written confirmation from a witness regarding the birth; if there is no witness, a sworn statement about the birth must be provided.

A written agreement between parents regarding the choice of nationality for their child.
If parents agree to choose a foreign nationality for their child, this agreement must be certified by the competent state authority of the country of which either parent is a citizen.
