2. The permanent residence registration dossier for persons specified in Clause 2, Article 20 of this Law includes:
a) A declaration form for changes to residence information, clearly stating the consent of the head of the household, the legal owner of the residence, or an authorized representative for permanent residence registration, except in cases where written consent has already been given;
b) Documents proving personal relationship with the head of the household or a household member, except in cases where information showing this relationship is already available in the National Population Database or the Residence Database;
c) Documents proving other conditions stipulated in points b and c of Clause 2, Article 20 of this Law.
