The current Party Member Card Issuance Form for 2025

The Party Member Card Issuance Register form currently used nationwide is Form 8-TĐV, issued with Guidance 38-HD/BTCTW. This is the standardized form for recording information on issuing Party membership cards to Party members in grassroots Party organizations.

Instructions for recording Party Membership Card Issuance in accordance with regulations.

The contents of the register are divided into two parts: party members who received their cards at the party branch and party members who received their cards from other party branches. Each column in the register has specific recording requirements as follows.

1. Party members receive their membership cards at the party committee.

This section has 6 columns. Each party member writes in one cell with two lines.

Column 1: Serial number

Record the numbers in ascending order according to the number of party members who have been issued membership cards by the party committee.

Column 2: Decision date and party membership card number

Top line: records the date the decision to issue the party membership card was made.

Bottom line: record the party membership card number issued.

Column 3: Full name and date of birth

Top line: Write the full name of the party member in capital letters, for example: VU VAN BIEN.

Bottom line: records the party member's date of birth.

Column 4: Date of joining the Party and date of official recognition

Top line: Date of joining the Party.

Bottom line: Date of being recognized as a full party member.

Column 5: Card issuance and signature confirmation

Top line: Name of the Party organization that issued the Party membership card.

Bottom line: Name of the party organization that signed the card.

Column 6: Party membership card status

Record cases such as: leaving the Party, death, loss of membership card, transfer of Party membership to another commune-level or equivalent Party committee.

2. Party members who received their membership cards at another party branch can transfer them to this branch.

For party members whose membership cards were issued by other party branches and transferred here, please record them in this separate section.

Column 1: Serial number according to the time of transfer

The numbering is based on the time the party member transferred to their current party branch.

Columns 2, 3, and 4: Fill in the same information as for party members receiving membership cards at the party committee.

This includes information about the decision date – card number, full name – date of birth, date of joining the Party – official date.

Column 5: Name of the Party Committee that initially issued the card

Clearly state the name of the party branch where the party member was issued their membership card before transferring.

Notes on recording in the Party Member Card Issuance Register

The information should be recorded continuously, without leaving any gaps on the same page or line breaks, to avoid omissions or confusion.
