The Party Member Profile Supplement Form is used to update changes in a Party member's information during their membership. Below is the latest template and detailed instructions for filling it out.

The current Party member profile supplement form, designated as mẫu 3-HSĐV, is issued in conjunction with Guidance 38-HD/BTCTW. This is an important document for updating information changes of each Party member throughout their Party membership.

Instructions on how to fill out the Party Member Profile Supplement Form according to regulations.

According to Section 2.3 of Guidance Document 38-HD/BTCTW, the Central Committee of the Party stipulates the following procedure for filling out the Party Member Profile Supplement Form.

1. List the items in the heading section.

The title section is filled out the same way as on the Party Membership Form. Party members must fill in the full name of the Party committee, branch, Party member's personal information number, Party member card number, and other identifying information according to the issued form.

2. Record the items in the content section.

For items with changed information, party members should clearly state the changes. Items that remain unchanged should be marked with the letter "K".

For sections already instructed on the form, fill in as directed. For the remaining sections, follow the instructions applied when filling out the Party Membership Form.

Procedures for using and managing the Party Member Profile Supplement Form

The Party branch committee or branch is responsible for guiding, receiving, and verifying the completeness of the Supplement Form. After confirmation, the Party branch committee records the changes in the list of Party members and forwards the Form to the grassroots Party committee. If the Party branch is a grassroots Party branch, the Party branch committee confirms the changes in the section designated for the grassroots Party committee.

The grassroots Party Committee adds the new information to the Party member's profile and the Party member list of the Party committee, and forwards the form to the immediate higher-level Party committee.

The immediate superior Party committee continues to check, receive, and update changes in the Party Member Form, Party Member Profile, Party Member List, and Party Member Database. The supplementary form is stored with the Party member's file. If the Party member's file is managed by the grassroots Party committee, the form is returned to the grassroots Party committee for safekeeping.
