On the afternoon of July 22nd, a delegation from the Ministry of Health, led by Professor Nguyen Tran Hien, Head of the National Expanded Immunization Project Management Board, held a working session with leaders and relevant agencies of Quang Tri province to investigate the cause of death of three newborns after receiving the Hepatitis B vaccine at Huong Hoa District General Hospital, Quang Tri province.

Following a closed-door professional meeting, the Director of the Quang Tri Provincial Department of Health, Tran Van Thanh, announced the initial conclusions of the Council. Accordingly, the cause of death for the three newborns was anaphylactic shock of unknown origin. The working group requested that samples of the vaccine and other samples (blood, lung tissue, brain, liver, kidney, heart) be sent to national and international laboratories for testing to verify the quality of the vaccine and any foreign substances present in the samples.
According to the Council's conclusions, approximately 600,000 doses from these two batches have been used nationwide without any reported adverse reactions. The vaccines were tested and have a release certificate from the National Institute for Vaccine and Biological Products Testing. The inspection team determined that vaccine management at the hospital did not comply with regulations; daily records were not kept; empty vials were not stored as required; and injections were administered in patient rooms instead of in a separate injection room.
