Nghe An Obstetrics and Pediatrics Hospital saves premature baby with abdominal wall defect and external organs
(Baonghean.vn) - Doctors at Nghe An Obstetrics and Pediatrics Hospital have just performed surgery to save the life of a premature infant with congenital abdominal wall defects, internal organs located outside the body, and intestinal malformations.
Accordingly, the baby's mother is pregnant woman TNQN (permanent resident of Huong Khe district, Ha Tinh province) who is pregnant for the second time. During her pregnancy, the mother did not go for regular check-ups or ultrasound screening. On March 22, the mother gave birth to a premature baby boy (35 weeks old), underweight (2.4 kg) at the district hospital. After birth, doctors discovered that the baby had no abdominal wall muscles or skin, and showed signs of abdominal wall hernia (the entire intestine was outside the abdomen). The patient was quickly transferred to Nghe An Obstetrics and Pediatrics Hospital.

The child was admitted to the hospital with malnutrition, weakness, hypothermia, signs of dehydration, respiratory failure, and a poor prognosis. The abdominal wall defect caused the child's stomach, entire small intestine, and colon to protrude out of the abdominal cavity. The child's stomach and small intestine were dilated and abnormally deformed; most of the digestive tract from the duodenum to the rectum was of unclear shape, and the intestinal loops had an abnormal structure.

The Intensive Care Surgery doctors quickly consulted with the Department of Surgery. Here, the patient was assigned comprehensive care, warming, intensive resuscitation, mechanical ventilation, intravenous nutrition and necessary paraclinical tests. After 2 hours of intensive resuscitation, seeing that the child's condition was stable, the doctors decided to perform surgery to suture the sterile bag for the child. Ten days later, the child's entire intestines were in the abdomen, and the patient was scheduled for a second surgery to close the abdominal wall. After the surgery, the child received special care in the Intensive Care Surgery Department, ensuring sterile procedures.
Doctors at Nghe An Obstetrics and Pediatrics Hospital said: This is a special surgery because premature babies and low birth weight babies often have poor organ function (heart, lungs, liver, kidneys, etc.), not yet fully developed... Therefore, the pressure on the surgical team is very high, requiring very high precision, especially for anesthetics, intravenous fluids... when injected into the baby's weak body, it must be accurate to the milliliter, otherwise the risk of cardiac arrest and respiratory arrest right on the operating table is very high. At the same time, the post-operative recovery process of the child is extremely difficult, the child continues to get worse, the surgical wound is tense due to the large area of intestine, difficult to heal, and severe pneumonia.
Faced with the child's critical condition, which seemed insurmountable, the doctors and nurses spent many sleepless nights adjusting the blood pressure to save the child's life. Up to now, after 2 months of intensive care and effective treatment, the child has recovered well, his digestive tract is clear, his general condition is stable, and he has been discharged from the hospital with a follow-up appointment after 1 month./.
