The expert advisory board for evaluating adverse events after vaccination held an assessment meeting with the participation of all members. In addition, there were 2 experts from the National Institute of Hygiene and Epidemiology and the National Children's Hospital.
At the meeting, the Council assessed the case of Lo Tuan Truong's death based on the results of the investigation and detailed analysis according to 4 groups of causes that may be related to the post-vaccination reaction: Errors due to vaccines and vaccination; Due to the body's reaction to the vaccine; Coincidence with other diseases; Unknown cause.
After synthesizing the conclusions according to each group of causes, the Council unanimously concluded as follows: The vaccine used for Lo Tuan Truong was provided by the Expanded Immunization Project, the process of receiving, transporting, preserving and using it was in accordance with regulations. No errors were detected in the vaccination process. The commune health workers promptly responded to the emergency response according to the protocol as soon as the incident occurred. Temporarily stop using and promptly inspect the vaccine batch related to the incident. Lo Tuan Truong, 3 months old, died after being vaccinated with Quinvaxem vaccine, most likely due to anaphylactic shock.
