The expert advisory board for post-vaccination adverse events held an assessment meeting with the full participation of all members. In addition, there was the participation of two experts from the National Institute of Hygiene and Epidemiology and the National Children's Hospital.

At the meeting, the Council assessed the case of Lo Tuan Truong's death based on the results of the investigation and detailed analysis according to 4 groups of causes that may be related to post-vaccination reactions: Errors due to vaccines and vaccination; Due to the body's reaction to the vaccine; Coincidence with other diseases; Unknown cause.

After synthesizing the conclusions according to each group of causes, the Council unanimously concluded as follows: The vaccine used for Lo Tuan Truong was provided by the Expanded Immunization Project, the process of receiving, transporting, preserving and using was in accordance with regulations. No errors were detected in the vaccination process. The commune health staff promptly handled the emergency according to the protocol as soon as the incident occurred. Temporarily stop using and soon inspect the vaccine batch related to the incident. Lo Tuan Truong, 3 months old, died after being vaccinated with Quinvaxem vaccine, most likely due to anaphylactic shock.
