The purpose of making a list of household health insurance participants is: Counting household members whose names are in the household registration book, temporary residence book or temporary residence certificate in the province who are participating in health insurance, have participated and have not participated in health insurance; fully and accurately declaring all household members on the health insurance participation declaration form to grasp the number of people in the province, the number who have participated in health insurance, the number who have not participated to serve as a basis for managing and monitoring health insurance participants and having solutions to mobilize people who have not participated to participate in health insurance according to the roadmap towards universal health insurance by 2020; reviewing all subjects participating in health insurance to avoid issuing duplicate health insurance cards.

The declaration of household members participating in health insurance is very important for the management of health insurance participants in the province, contributing to accurately grasping the coverage rate of people with health insurance cards/population. Therefore, there must be close coordination between relevant departments, branches and People's Committees of districts, cities and towns from the beginning to the end of this list making work.
To ensure the list is accurate and on time, households are responsible for fully and accurately declaring all household members on the List of Households Participating in Health Insurance (DK01) as a basis for managing and monitoring health insurance participants. Form DK01 is received by the heads of the residential areas at the households and handed over to the People's Committee at the commune level.

From the indicators listed in form DK01, the People's Committee at the commune level assigns staff to synthesize, review and enter information into the management software; classify subjects according to the List of declarations of people participating in health insurance (Form DK02) and the List of declarations of people not participating in health insurance (Form DK03) and then transfer this information to the Social Insurance (SI) at the district level.
Based on the data sent by the People's Committee at the commune level, the district-level Social Insurance agency classifies and synthesizes according to each type of subject; transfers form DK03 to the collection agent and makes a list to compare and confirm health insurance participants with the subject management agencies (DK06).
After consulting with the District People's Committee, the District Social Insurance Office shall establish a Steering Committee and issue an implementation plan; proactively coordinate with the Provincial Social Insurance Departments to organize training for the People's Committees of communes, wards and towns; group leaders of areas and hamlets; collection agents (of the Post Office and the Commune People's Committee). Coordinate with the District Post Office to disseminate information to the collection agent staff to participate in collecting information and transferring documents related to the list of households participating in health insurance.
District-level Social Insurance provides funding to commune-level People's Committees to support the establishment of a list of households participating in health insurance and make payments according to regulations.
The district-level Social Insurance assigns each health insurance collection officer to be in charge of 5 to 10 communes, wards, and towns (depending on the locality), directly coordinating with the People's Committee at the commune level and each hamlet and residential group to ensure the implementation of the list of households participating in health insurance is on schedule and effective.
The district-level Social Insurance is responsible for making a list, providing the declaration form for household members participating in health insurance, and providing instructions on how to use the management software to make a list of households participating in health insurance for the commune-level People's Committee. The implementation of making a list of households participating in health insurance and entering data will start from November 1, 2015 and be completed before December 31, 2015.
