Acute form: Due to wind-heat and heat toxins invading the liver and gallbladder. The patient has symptoms of fever, chills, headache, tinnitus, ear pain, thick yellow ear discharge with blood, yellow tongue coating, and a deep and rapid pulse. Treatment: dispel wind and clear heat; eliminate damp heat in the liver and gallbladder meridians. Use one of the following prescriptions:
Recipe 1: Modified Saiko Thanh Can Decoction: Saiko, Gentian, Scutellaria, Gardenia, Arctium, Arctium, each 12g; Mint 6g, Lonicera japonica 20g. If there is bleeding and pus, add 16g Rehmannia glutinosa, 12g Danpi. Boil and drink 1 dose per day.

Phellodendron bark (charcoal bark of Phellodendron tree)
Recipe 2: Long dom ta can thang with modifications: long dom thao, scutellaria baicalensis, mu thong, sinh dia, sa tien tu, trach ta, each 12g; gardenia fruit 8g, angelica 8g, licorice 4g. If there is high fever, thick pus with blood, red face, red tongue, add 16g honeysuckle, 12g forsythia.
If constipated, add 6g rhubarb. If fever is low, ear pain is severe, and pus is small, remove rehmannia root and add 16g coix seed, 6g cyperus rotundus, 6g atractylodes macrocephala, and 6g atractylodes macrocephala. Boil and drink 1 dose per day.

Chronic form: if the pain persists without fever, it is due to kidney fire deficiency; if the pain persists with digestive symptoms such as poor appetite, weight loss, and diarrhea, it is due to spleen deficiency and damp heat. In treatment, it is divided into 3 forms as follows:
Liver meridian damp heat syndrome: The patient has symptoms of ear pain, thick, sticky pus, foul smell, and large amount. Treatment: clear the liver and benefit dampness. Use the prescription Long dom ta can thang (as above).
Kidney deficiency and inflammation: Patients have symptoms of frequent ear discharge, thin pus, tinnitus, hearing loss, dizziness, little sleep, back and knee pain, thin and rapid pulse. Treatment: nourish yin and clear heat, nourish the kidney and clear the orifices. Use one of the following formulas:
Recipe 1: Tri ba dia hoang thang: 12g rehmannia root, 16g Chinese yam, 8g each of cornus officinalis, alisma, peony bark, poria, anemarrhena, phellodendron bark. Boil and drink 1 dose per day. Or grind into powder and make pills, drink 18g per day, divided into 3 times.

Codonopsis pilosula
Lesson 2: Great Yin Tonic Pill: Phellodendron bark 12g, Anemarrhena rhizome 12g, Rehmannia glutinosa 16g, Turtle shell 16g. Boil and drink 1 dose per day, or make pills, drink 16g per day divided into 3 times, drink long term.

Recipe 3: Modified version of Bo trung ich khi thang: 12g codonopsis pilosula, 12g astragalus, 12g bupleurum, 8g cimicifuga, 12g atractylodes macrocephala, 8g angelica, 4g licorice, 6g tangerine peel, 8g phellodendron, 8g coptis, 12g poria.
