A serious control system failure that occurred during the flight caused the UH-1 helicopter, serial number 7912, to crash.

On January 31, the Air Defense - Air Force Service and Air Division 370 held a conference to teach flight safety and announced the conclusion of the level 1 aircraft accident of Regiment 917, Air Division 370.

Accordingly, the cause of the accident was determined to be during a flight at an altitude of 300m, the plane suddenly lost control (possibly the control system was broken or jammed), causing the plane to dive at a large angle, speed and tilt.

The flight crew made great efforts to bring the plane back to level flight but could not handle it (due to high speed and rapid loss of altitude), leading to the accident.

Previously, on the morning of January 28, the UH-1 training aircraft of Regiment 917, Air Division 370 took off from Tan Son Nhat airport at 7:15 a.m. and lost contact with the ground at 7:23 a.m.

Later, this helicopter was determined to have crashed in Hamlet 4, Pham Van Hai Commune, Binh Chanh District.

All four crew members died at the scene of the accident.
