Resolution No. 21-NQ/TW dated November 22, 2012 of the Politburo on strengthening the Party's leadership over social insurance and health insurance work for the 2012-2020 period

I- SITUATION AND CAUSES

1- Social insurance and health insurance are two major policies of the Party and the State. In recent years, social insurance and health insurance have achieved important achievements, contributing to stabilizing people's lives, implementing social justice and socio-political stability. The system of policies and laws on social insurance and health insurance has been gradually improved in line with the country's socio-economic development; the number of people participating in social insurance and health insurance has increased over the years; pensions and social insurance regimes have been paid in accordance with the provisions of law; the rights of health insurance participants have been increasingly expanded. The social insurance fund has been formed, has a surplus and is preserved, growing, and participating in investments to contribute to socio-economic development. The health insurance fund has initially balanced revenue and expenditure and has a surplus. The Vietnamese social insurance organization system has been formed and developed, basically meeting the requirements of implementing social insurance and health insurance regimes and policies.

2- However, the work of social insurance and health insurance still has some limitations and weaknesses. The coverage of social insurance is still low, reaching only about 20% of the workforce, the number of people participating in health insurance is only about 65% of the population. State management of social insurance and health insurance has not met the requirements; the organization and implementation of social insurance and health insurance regimes and policies still has shortcomings. The situation of enterprises owing and evading social insurance and health insurance payments is still common. Management and use of the Social Insurance Fund is not strict, there are cases of lending to the wrong subjects. The Social Insurance Fund, especially the Pension and Death Fund, is at risk of imbalance in the near future; the situation of workers abusing unemployment insurance policies is quite common. The Health Insurance Fund always has the potential risk of deficit; medical examination and treatment under health insurance has not met the needs. The service attitude of some medical staff and administrative procedures in medical examination and treatment still cause frustration for patients; the capacity and responsibility of some medical examiners are still weak.

3- The main reason for the above limitations and weaknesses is that many Party committees and authorities have not really paid attention to leading and directing the implementation of social insurance and health insurance policies and regimes. The coordination between state management agencies and local social insurance agencies in implementing social insurance regimes is not tight. Inspection and examination of the implementation of social insurance and health insurance laws is still weak. The social insurance and health insurance system has not been modernized and is still unprofessional, not proactive in reaching out to employees and employers. The work of propagating and disseminating policies and laws on social insurance and health insurance has not received due attention and is not very effective.

II- VIEWPOINT

1- Social insurance and health insurance are two important social policies, the main pillars of the social security system, contributing to social progress and equity, ensuring political and social stability and socio-economic development.

2- Expand and improve the social insurance and health insurance regimes and policies with steps and roadmaps in line with the country's socio-economic development. Develop the social insurance and health insurance system in sync with the development of social services, increasingly better meeting the people's needs; create favorable conditions for everyone to participate in and enjoy social insurance and health insurance regimes.

3- Social insurance and health insurance must follow the principle of contribution and benefit, with rights corresponding to obligations and sharing among members, ensuring fairness and sustainability of the social insurance and health insurance system.

4- Well implementing social insurance and health insurance policies and regimes is the responsibility of party committees at all levels, authorities, unions, social organizations, enterprises and each citizen.

III- OBJECTIVES

Effectively implement social insurance and health insurance policies and regimes; rapidly increase the coverage of social insurance participants, especially voluntary social insurance; achieve the goal of universal health insurance. Strive to have about 50% of the workforce participating in social insurance by 2020, 35% of the workforce participating in unemployment insurance; over 80% of the population participating in health insurance. Safely use and ensure the balance of the Social Insurance Fund in the long term; effectively manage, use and ensure the balance of the Health Insurance Fund. Build a modern, professional, highly effective social insurance and health insurance system to meet the requirements of industrialization, modernization and international integration.

IV- TASKS AND SOLUTIONS

1- Strengthen leadership and direction of propaganda and dissemination of laws, policies and regimes on social insurance and health insurance.

Party committees and authorities at all levels need to clearly define their responsibilities in leading and directing the implementation of social insurance and health insurance policies and regimes. Strengthen leadership in disseminating and popularizing laws, policies and regimes on social insurance and health insurance so that cadres, party members and people can clearly see the role and significance of social insurance and health insurance in the social security system; the rights and obligations of people when participating in social insurance and health insurance. Timely detect and commend localities, units, organizations and enterprises that perform well, and at the same time criticize and strictly handle violations. Authorities, social insurance agencies and mass organizations need to pay attention to doing a good job of mobilizing people to participate in social insurance and health insurance.

2- Perfecting the system of policies and laws on social insurance and health insurance

Research and amend the Law on Social Insurance in the direction of expanding the number of social insurance participants. Complete policies, laws, management and use mechanisms of the Social Insurance Fund, ensuring the balance and growth of the Fund. Review and supplement regulations requiring employers to properly and fully perform their obligations to pay social insurance for employees. Amend unemployment insurance policies to ensure the legitimate rights of unemployed employees and avoid insurance fraud.

Amending and supplementing the Law on Health Insurance; studying and amending the policy on regulating the surplus health insurance revenue from localities to the central government, reserving an appropriate proportion for localities to improve the quality of medical examination and treatment; improving the effectiveness and efficiency of state management of health insurance. Having policies to encourage people, especially those with incomes below average, to participate in health insurance. Improving the effectiveness of health insurance use for ethnic minorities, mountainous areas, and poor households.

Early promulgation of the Strategy for the development of social insurance and health insurance until 2020. Step by step implementation of the "contribution - benefit" principle, associated with adjusting the retirement age in certain fields and occupations in accordance with the socio-economic development conditions of the country.

Expand the State's support policy for participants in social insurance and health insurance, especially voluntary social insurance for farmers and workers in the informal sector; health insurance for policy beneficiaries, people in near-poor households, farmers, students, and people in particularly difficult and mountainous areas. Apply new types of social insurance suitable to people's needs. Pilot a supplementary pension insurance policy.

Diversify health insurance products and services to suit all social classes, adjust fees and benefits along with developing high-quality health services. Have policies to encourage people to participate in long-term voluntary health insurance, limiting the situation of people only buying health insurance when they are sick. Regulate health insurance payment levels in a way that both ensures the rights of insurance participants and ensures fund balance according to the "pay - enjoy" principle.

3- Improve service quality and implementation of social insurance and health insurance policies and regimes

Innovate the operation method of the social insurance and health insurance systems towards quick and timely payment, ensuring the rights of insurance participants.

Implement transparency, publicity and strengthen management of medical examination and treatment activities to limit and prevent health insurance fraud, abuse of high technology and expensive medicines.

Improve the organization of social insurance at all levels to implement social insurance and health insurance policies and regimes to meet increasing requirements. Promote administrative procedure reform, improve service quality, and reduce inconvenience for social insurance and health insurance participants.

Improve the capacity of cadres, civil servants and public employees in the fields of social insurance and health insurance; expand international cooperation, promote the application of information technology, and gradually modernize the social insurance and health insurance management system.

4- Strengthening state management of social insurance and health insurance

Strengthen close coordination between state management agencies and social insurance agencies at all levels in implementing social insurance and health insurance regimes and policies. Manage well the subjects contributing and participating in social insurance and health insurance from the grassroots level. Strengthen and enhance the management of the Social Insurance Fund and Health Insurance Fund to ensure revenue, expenditure, and effective and safe investment and growth of the Fund in accordance with the law.

Strengthen inspection and examination of compliance with laws on social insurance and health insurance; promptly detect and handle violations, especially acts of evasion and debt of social insurance and health insurance premiums and negative and fraudulent acts to enjoy benefits and take advantage of social insurance and health insurance.

V- IMPLEMENTATION ORGANIZATION

1- Provincial and municipal Party Committees, Party committees, Party executive committees, Party delegations, and Party committees directly under the Central Committee organize the study and dissemination of the Resolution; develop programs and plans to implement the Resolution.

2- The National Assembly Party Delegation and the Government Party Committee direct the amendment, supplementation and completion of laws on social insurance and health insurance.

3- The Central Propaganda Department presides over and coordinates with the Ministry of Information and Communications, the Ministry of Labor, War Invalids and Social Affairs, the Ministry of Health, the Ministry of Justice, the Vietnam Social Security, the Vietnam General Confederation of Labor, the Vietnam Chamber of Commerce and Industry, the Vietnam Cooperative Alliance, and the Vietnam Farmers' Union to organize propaganda and dissemination of Resolutions, laws, policies, and regimes on social insurance and health insurance.

4- Local party committees lead the people's committees to closely coordinate with social insurance agencies to effectively implement social insurance and health insurance in the area. Every year or when necessary, party committees work with social insurance agencies on the implementation of social insurance and health insurance policies and regimes.

5- The Central Party Office shall preside over and coordinate with the Party Committee of the Ministry of Labor, War Invalids and Social Affairs, the Party Committee of the Ministry of Health and Vietnam Social Security to guide the implementation, inspect and evaluate the implementation of the Resolution and periodically report to the Politburo./.
