Nghe An Newspaper reporters had an interview with Mr. Bui Dinh Long - Director of the Department of Health to clarify this issue.

PV: Một số bạn đọc phản ánh rằng, kết quả xét nghiệm, chẩn đoán có sự khác nhau giữa tuyến huyện và tuyến tỉnh nên dẫn đến khi ở tuyến huyện chẩn đoán một bệnh, đến Bệnh viện Đa khoa tỉnh lại phát hiện ra bệnh khác, xin ông cho biết đâu là nguyên nhân dẫn đến việc này?

Mr. Bui Dinh Long: Kết quả xét nghiệm, chẩn đoán bệnh phụ thuộc vào nhiều yếu tố, như xét nghiệm máu phụ thuộc vào thời gian lấy máu, điều kiện lấy máu… Các loại bệnh cũng có diễn biến theo từng thời điểm. Ngoài ra còn phụ thuộc vào những yếu tố khách quan, một số chủ quan khác như: kỹ thuật lấy máu cũng ảnh hưởng, ngay cả hai loại máy xét nghiệm khác nhau cũng có chỉ số khác nhau, nhưng sự khác biệt là không đáng kể.

PV: Đã có nhiều trường hợp bệnh viện huyện bảo có sỏi, lên tuyến tỉnh lại bảo không có sỏi?

Mr. Bui Dinh Long: Việc này cũng phụ thuộc vào nhiều yếu tố

PV: Do trình độ bác sỹ hay do thiết bị?

Mr. Bui Dinh Long: Do trình độ bác sỹ là chủ yếu, ngoài ra cũng do ảnh hưởng sự di chuyển của viên sỏi, có khi nằm ở thận quá trình người bệnh vận động nó lại chạy lên niệu quản, từ niệu quản lại chạy lên bàng quang.

PV: Một số ý kiến cho rằng, chất lượng khám chữa bệnh ở tuyến huyện kém hơn hẳn so với Bệnh viện Đa khoa tỉnh, ông có ý kiến gì về vấn đề này?

Mr. Bui Dinh Long: Với những bệnh thông thường giữa tuyến huyện và tuyến tỉnh không có sự khác biệt nhiều, nhưng những bệnh khó, chuyên sâu thì sự khác biệt là đương nhiên. Vì bệnh viện tuyến huyện đa số chỉ là bệnh viện hạng 3, bệnh viện tuyến tỉnh là bệnh viện hạng 1, cách nhau rất nhiều về năng lực chuyên môn. Bệnh viện tuyến huyện có khoa nhưng chỉ ở mức độ chuyên khoa rộng, còn ở bệnh viện tuyến tỉnh thì chuyên khoa sâu. Thí dụ ở bệnh viện tuyến huyện khoa ngoại - sản thì làm cả sản, cả ngoại lẫn chấn thương, lên ở tuyến tỉnh có ngoại tiêu hóa, ngoại tiết niệu…

In some district hospitals, only the first cesarean section is allowed, but for 2-3 cesarean sections, the patient must be transferred to the province...

People's request for district hospitals to be like provincial hospitals is difficult, not possible. Serious or minor illnesses must be classified. District hospitals have only invested in examining and treating common illnesses.

PV:So in some common cases like broken arms, broken legs, plaster casts at the district hospital also have to be re-casted, is it due to the doctor's level?

Mr. Bui Dinh Long:When a bone is broken, the position may be misaligned, and it is difficult to set it once, even with a local anastomosis. It may also be due to the doctor's skill or pathological factors.

PV:Patients who use health insurance cards for medical examination still feel discriminated against?

Mr. Bui Dinh Long:This is due to the patient's feelings, because currently most of the medical examination and treatment are done with health insurance cards. There are hospitals where about 90% of patients are examined with health insurance cards. Examination, treatment, and prescription for people with health insurance cards are always easier than those who use services because they only have to pay 15-20% of the amount.

PV:According to you, what is the current medical ethics problem in Nghe An?

Mr. Bui Dinh Long:The issue of medical ethics today depends on how we evaluate it. In Nghe An, we have also made many efforts, although there are still factors that patients think need to be overcome. Nghe An is also the only province with a medical ethics project that has been implemented since 2004. Compared to before, the medical sector has more specific standards to strive for, however, each person's perception is different.

PV:Currently in our province, there is still a situation where health certificates can be easily purchased without examination?

Mr. Bui Dinh Long:The issuance of Health Certificates has been clearly regulated by the Ministry of Health, according to which only district level hospitals of grade 3 or higher are allowed to examine and issue. Recently, there were some clinics that were joint ventures with some driving training centers to issue them, that was wrong and we have checked, corrected and banned it. Now the Health Certificate has full specialties so only hospitals can examine.

PV:But according to feedback, at some hospitals, people still "ask" for health certificates?

Mr. Bui Dinh Long:There are many factors to this, and we are currently working on it.

PV:What advice do you have to help people feel secure in their initial medical examination and treatment?

Mr. Bui Dinh Long:When you detect abnormalities in your body, you should go for a medical check-up. Early detection of disease makes it easier to treat.
