Recently, Nghe An Friendship General Hospital successfully saved a Laotian patient who was at high risk of death due to septic shock, multiple organ failure, urinary tract infection, and ARDS pneumonia.
On February 25th, the Intensive Care Unit,Nghe An Friendship General HospitalWe received patient Aloun Sack, born in 1979. The patient resides in Khamkeuth district and currently works in Viengthong district, Bolikhamsai province, Lao People's Democratic Republic.

The patient was admitted with the following diagnosis: Circulatory collapse of unknown cause; history of bilateral kidney stones treated with extracorporeal shockwave lithotripsy 7 years prior; intermittently treated hypertension.
Three days prior, patient Aloun Sack had a fever and felt very tired. On February 25th, the patient experienced decreased consciousness, low blood pressure, and rapid breathing; first aid was administered at a district-level hospital in Laos, after which the patient was transferred to Vietnam.

Before the patient arrivesNghe An Friendship General HospitalRegarding treatment, the Nghe An Department of Health and the hospital received a request for medical assistance from the Bolikhamsai Provincial Department of Foreign Affairs for the case of patient Aloun Sack.
Upon admission, patient Aloun Sack was in critical condition, with impaired consciousness, rapid and shallow breathing, pale lips and extremities, low blood pressure, decreased urination, dry skin, and elevated blood lactate levels.
Doctors at the Emergency Department of Nghe An Friendship General Hospital intubated the patient, put him on a ventilator, maintained sedation and high-dose vasopressors, performed abdominal and chest CT scans, consulted with specialists, and transferred him to the intensive care unit for further treatment.

The patient was at high risk of death. Under the chairmanship of Dr. Pham Hong Phuong, Deputy Director of Nghe An Friendship General Hospital, the Intensive Care Unit organized a hospital-wide consultation with specialists from various departments.
Doctors diagnosed the patient with septic shock, multiple organ failure (SOFA score 12), urinary tract infection, and ARDS pneumonia. Treatment included enhanced lung-protective mechanical ventilation, sedation, maintenance of two high-dose vasopressors, arterial catheterization for continuous blood pressure monitoring, non-invasive hemodynamic monitoring, fluid resuscitation, continuous hemofiltration combined with adsorption hemodialysis, comprehensive care, and parenteral nutrition.

After 7 days of intensive treatment, patient Aloun Sack was taken off continuous hemodialysis, mechanical ventilation was discontinued, and the endotracheal tube was removed. Cardiac and respiratory functions stabilized, kidney function recovered well, the patient's overall condition improved, and he was eligible for discharge. By the morning of March 11th, patient Aloun Sack had fully recovered and was discharged from the hospital.
The success of this case demonstrates the effectiveness of simultaneously applying advanced, specialized techniques in the diagnosis, monitoring, and treatment of critically ill patients at Nghe An Friendship General Hospital; opening up many opportunities to save the lives of critically ill patients in the North Central region and neighboring Laos.
