Nghe An: Saving the life of a patient with rare transverse colon necrosis
(Baonghean.vn) - Nghe An General Hospital has just saved the life of patient Nguyen Hong Q. who suffered from septic shock due to herniation of the transverse colon into the inguinal canal, necrosis, and general peritonitis.
Patient Nguyen Hong Q. (76 years old, Nghi Xuan, Ha Tinh) was admitted to the hospital on the evening of April 23 in a state of severe shock, rapid pulse, low blood pressure, purple veins all over the body, senile pneumonia, with a history of right inguinal hernia for many years. The prognosis is very serious.
Doctors from the Emergency Department, Surgical Resuscitation, Anesthesia and Resuscitation, and Gastroenterology Department quickly consulted and performed emergency resuscitation, and performed diagnostic paraclinical tests. The surgical team determined that patient Q. was in toxic shock due to a strangulated, necrotic, perforated right inguinal hernia. The surgical team urgently performed emergency surgery to save the patient's life.

“When opening the patient’s abdomen, we noticed an abnormality caused by the herniated organ through the inguinal canal causing perforation and necrosis, which was the transverse colon and the greater omentum. Normally, the right inguinal hernia is often the cecum or small intestine, while the left inguinal hernia is the sigmoid colon and small intestine.
In this case, the cause may be that the patient's transverse colon is too long. The entire abdomen is filled with foul-smelling fecal fluid flowing out from the colon perforation. We cleaned the entire abdomen, removed the necrotic colon, and brought the colon out to make an artificial anus. At the same time, we completely removed and sutured the inguinal canal," said Master Dinh Van Chien, Department of Digestive Surgery, the main surgeon of the surgery.
The post-operative recovery process for patient Q. also encountered many difficulties due to the patient's old age, pneumonia, and major surgery, so the patient was treated and nourished intravenously at the Department of Surgical Resuscitation and Intensive Care for Poison Control for more than a week. Currently, the patient's condition has stabilized, he can eat and walk.
Doctor Chien added: Inguinal hernia is a common surgical disease. In cases with signs of inguinal hernia such as: bulging in the groin when exerting (coughing, straining, carrying heavy objects...) and shrinking when resting, it is necessary to go to a medical facility for examination, diagnosis and early treatment to avoid serious complications as above.
