Urgently going down to the lobby on the first floor to wait, the obstetrics team on duty received and provided emergency care to the pregnant woman Vo Thi T. who was transferred from the local health facility in a state of shock and circulatory collapse due to severe blood loss, with a near risk of death.

Medical history information was quickly extracted: More than 1 hour earlier, at 6:00 am the same day, the mother gave birth to her fifth child weighing 3,800g at a lower-level hospital. After giving birth, the mother suffered from uterine atony and hemorrhage. Although the doctors at the district hospital treated her with emergency treatment to stop the bleeding, the mother still showed signs of continued blood loss and gradually fell into a state of shock.

Realizing the situation was critical, beyond control, and lacking the means and equipment to treat the patient, the lower-level hospital called for support.Nghe An Obstetrics and Pediatrics Hospitaland the pregnant woman was transferred to a higher level emergency room.

As soon as the patient T was admitted, the on-duty team consulted and agreed on the diagnosis: Ms. T. had severe anemia and circulatory collapse due to uterine atony bleeding, and needed emergency surgery to stop the bleeding and perform a blood transfusion to compensate for the loss.

The surgical team, including Dr. Tran Quang Hanh - Deputy Head of Obstetrics and Gynecology Department, Master Tran Xuan Canh - Deputy Head of Obstetrics and Gynecology Department, along with the anesthesia and resuscitation team, urgently performed emergency surgery. The plan to remove the uterus and suture the vagina was quickly carried out by the doctors to stop the bleeding.

At the same time, anesthesiologists, resuscitators, and surgical resuscitators tried to maintain blood circulation, mechanical ventilation, and blood pressure, and continuously mobilized blood type B from the hospital's blood bank and relatives to transfuse the patient. 25 people including doctors, nurses, and relatives donated blood to save the mother.

“After surgery, patienttemporarily out of danger. However, the difficulty is that the patient is in a state of postoperative coagulation disorder. The patient needs to be actively adjusted for coagulation disorder: platelet transfusion, freeze-dried factor VIII preparation. Under the direct guidance of the Board of Directors, the Department of Surgical Intensive Care has conducted consultations and continuously coordinated between specialties to provide the most accurate resuscitation adjustments for the condition of the patient T." - Dr. Tran Minh Long - Deputy Head of the Department of Surgical Intensive Care said.

Despite losing a lot of blood and having to transfuse 6,050 ml of blood from the outside (equivalent to 1.5 times of blood transfusion for the entire body), thanks to active resuscitation, the mother T. overcame the "doors of death", gradually stabilizing her health, stopping the ventilator, becoming alert, her skin and mucous membranes turning pink, and she was able to move lightly in the hospital bed.

The mother T. emotionally shared: “My husband and I have 4 children, this is the 5th birth. The previous times, I gave birth smoothly so I did not think I would fall into the situation of “life and death” postpartum hemorrhage. After waking up from the near-death experience, I feel lucky to still have a day to return to my children. I am extremely grateful to the doctors and nurses."
